He was predeceased by: his parents, Kate Mahaffey and Pierre Mahaffey; and his siblings, Pete, Dick and Mary Jane.
He is survived by: his former wife Kathleen Fields; his grandson Nate; his child Led; his wife Kathie Mahaffey; his daughters, Jill Mahaffey, Beth Mahaffey (David Anderson) and Anne M. Ladd (Paul Ladd); and his grandson Matthew Ladd. He is also survived by cousins, nieces and nephews.
