He was predeceased by: his parents, Grady T. Smotherman and Louise W. Smotherman. He is survived by: his wife Shirley Ervin Smotherman; his sons, Charles Smotherman (Sharon) and Tim Smotherman (Gena); and his grandchildren, Eric Smotherman (Paige), Katie Smotherman, Holden Smotherman and Madeleine Smotherman.

In lieu of flowers, donations may be made to the Parkinson's Foundation in memory of David.
