He is survived by: his spouse Rosie Ann Stewart Polk; his sons, Cedrick Lewis of Akins, SC and Patrick H. Polk, Jr. of Akins, SC; his brother Christopher Lewis; his friend Georgia Clemmons; his sisters, Martha Ann, Harris and Cathy Wilkerson; his mother-in-law Ida Daniels; his siblings-in-law, Patricia Daniels of Americus, GA, Tony Tyson of Americus, GA, Catrina Hawkins (Aaron) of Americus, GA and Carlos Reynolds; and also, Antonio Cannon, Donnie Lewis of Americus, GA, Rodney Polk, Christopher Slappey, Zachary Williams (Tonya) and Keith Williams. He is also survived by nieces, nephews, cousins other relatives and friends.
