He was predeceased by: his parents, Walter Putek and Frances Putek; and his brother Henry W. Putek Sr.. He is survived by: his wife Tricia Putek; his sister Dolores Dziadus (Stanley) of Chicago, Illinois; his sister-in-law Mary Lou Putek of Woolrich; and his nieces and nephews, Hank Putek, Jr., Gwen Echols, Joanne Williams, Janet Putek and Jon Putek.
