He was predeceased by: his parents, Ozell Cunningham and Frances Treadway Cunningham. He is survived by: his wife Janice Blake Cunningham; his son John Blake Cunningham (Katie); his grandchildren, Audrey and Henry Cunningham; and his siblings, Winton Cunningham, Mary Singleton, Anne Harris and G. David Cunningham.
