He was predeceased by: his parents, Amzie Cook and Goldie Cook; and his siblings, William "Lee" Cook, Yvonne Stevens and Anna Baker.
He is survived by: his daughter Lisa Cook of Kennesaw, GA; and his siblings, James Cook of Goose Creek, SC and Retha Davidson of Fenton, MO. He is also survived by numerous nieces and nephews.
