He was predeceased by: his parents, Jack Burbank and Helen Burbank; and his sister Dorothy Wroblewski. He is survived by: his close friend Carmen O'Kane; his children, Traci Clark (Jerry), Michael Burbank (Nancy Fellows) and April Carter (Peter); his grandchildren, Jerry L Clark II (Valerie), Tiffany Clark, Gavin Carter (Victoria) and Lindsay Smithdale (Richard); his great grandchildren, Max and Ruben Carter; and his siblings, John Burbank, Jacquelynne Moore (George) and Barbara Knopf (Don).
