She was predeceased by: her parents, Horace Creutz and Patricia Creutz-Cumbie (Doyle); her brother Pat Creutz (Cathy); her children, Richard Provost and Sandra Provost; and her granddaughter Nicole Provost.
She is survived by: her husband Richard A. Provost; her sons, Michael Provost (Sharon), Ronald Provost, Daniel Provost (Jonie), Timothy (Mary) and Christopher Provost (Kristine); her daughters-in-law, Gail and Carol; and her grandchildren, Rick, Adam, Derick, Ryan, Cory, Benjamin, Devon Provost, Meghan Mancuso (Provost), Amy Andraid (Provost), Melissa, Jennifer and Kelsie Provost. She is also survived by seven great-grandchildren; and several niece's and nephews.
