He is survived by: his close friend Blue; his fiancée Cheryl Freeman of Yulee, FL; his son Alvin David Butler, III; his granddaughters, Ella Rose Butler and Jana Guler of Atlanta, GA; and his siblings, Donald Butler (April) of Yulee, FL, Judy Allen of Big Canoe, GA, Ginger Hickox (Travis) and Jeanette Bryant of Jacksonville, FL.
Donations may be made in David's name to the Warner Community Hospice Center, 1348 S. 18th Street, Fernandina Beach, Florida.
