He was predeceased by: his wife Sally (Johnson); his parents, John William O'Leary and Helen (Walter); and his siblings, Ellen (Kuehl), Susan (Ahern) and Tom.
He is survived by: his sister Kathleen "Kate" (Fieweger); his children, John Wlliam O'Leary III (Lori), Erin Jane O'Leary (Anthony Garganese) and Ryan Patrick O'Leary (Amie); his grandchildren, Eimile, Isabella, Alessandra, John IV, Angelina, Patrick and Sean; and his close friend Christine Jorin.
