He was predeceased by: his wife Theresa (St Mary); and his parents, Joseph Fazio and Dora Fazio (Messica). He is survived by: his siblings, Doris Koharcheck, Marie Koch (Richard), Joseph Fazio (Judith) and Nick Fazio (Fiancee Julene Hussey).
