He was predeceased by: his parents, Mijo Poljak and Bara Simunic; and his son Richard Poljak. He is survived by: his granddaughter Michaela A. Asaro; his wife Renate Poljak; his son Christopher Poljak (Daniela); and his granddaughters, Julia R. Poljak and Jenna R. Poljak.

Visitation was held on Wednesday, January 22nd 2025 from 3:00 PM to 6:00 PM at the Aycock-Riverside Funeral and Cremation Center (1112 Military Trail, Jupiter, FL 33458). A funeral service was held on Wednesday, January 22nd 2025 at 5:00 PM at the same location.
