He was predeceased by: his sister Cathy Slayback. He is survived by: his son John Scott Slayback (Melissa); his close friend Beverly Levitt; his sisters, Cindy and Connie Slayback; and his grandchildren, Christian Scott Slayback, Alexis Levitt and Hopelynn McKinney.
