He was predeceased by: his parents, Oscar Polk and Etha Polk (Magdalena); and his siblings, Calvin, Cecil, Edgar, Floyd, Hubert Polk, Marie Velle and Hazel Ballou.
He is survived by: his daughters, Bettina Bairley, Belinda Rolicheck and Michele Polk; his significant other Virginia Albritton; his grandchildren, Nicholaus, Brittany, Ben, Sam, Maggie and Sophia; and his great grandchildren, Grayson, Xavier, Jennifer and Dagny.
