He was predeceased by: his parents, Anna Kafafian Formica and Frank Formica, Sr.; his wife Gaye; and his son Jeffrey Formica. He is survived by: his children, Gregory Formica and Lizanne Hart (Paul); and his grandchildren, Katie Hart, Ryan Hart, Brooke Formica and Olivia Formica.
