He was predeceased by: his father Chad Stephenson. He is survived by: his mother Tessa Cornell; his stepfather Justin Cornell; his brothers, Dylan Cornell and Wyatt Cornell; and his grandparents, Teresa, Jimmy Jones, Bev, Rick Campbell and David Stephenson.

In remembrance of Chase's loving and giving spirit, the family asks that any charitable donations be made, in lieu of flowers, to St. Jude's Children's Research Hospital (Memorial Giving, 501 St. Jude Place, Memphis, TN 38105).
