He is survived by: his wife Rose Marie Forsythe; his children, Karen Thompson, David Forsythe (Tami) of Ft. Myers, FL and Lee Forsythe (Cherie) of Illinois; and his sisters, Phyllis Smith and Dorothy Conry. He is also survived by 3 grandchildren and 2 great-grandchildren.
