He was predeceased by: his parents, Grover Faulk and Sally Faulk; and his sister Judith Zanath. He is survived by: his children, Amy Wylie (Jim) of Aliquippa, PA and Steven Faulk of New Sewickley, PA; and his stepchildren, Ashleigh Welborn (Bastian), Donald Haworth of Ft. Collins, CO, Bradley Haworth (Samantha) of Schuylkill, Austin Haworth (Cheyenne) of Houghton Lake, MI and Glenn Haworth of Titusville.
