He was predeceased by: his former wife Ethyl (Carey); his parents, Samuel and Sarah; his siblings, Jane, Charles and William; and his grandson Jeremiah.

He is survived by: his wife Pine Haven Campground; his children, Karen Sylvester (Steve) and David, Jr.; his brother Richard; his grandchildren, Stephen Sylvester, Jr., Jacob Buckwalter and Stacy Donohue (Kevin); and his great grandchildren, Madelyn, Connor Donohue, Roman and Wyatt Buckwalter. He is also survived by his parents, 3 brothers and a sister.
