He was predeceased by: his parents, Jack Sr. and Lillian. He is survived by: his wife MaryJane; his sons, Jack and Mark; his children-in-law, Hille and Shannan; his grandsons, Gavin, Andrew and Jackson; his sister Judy Hall (Bob); and his son Brian Merrill. He is also survived by many nieces, nephews and cousins.

In lieu of flowers, the family requests donations to the Moffitt Cancer Center, St Jude's Children's Hospital, or the Humane Society of Tampa Bay.
