He is survived by: his wife Angela Cook; his children, Jessica Cook, Jesse Lee Cook, Joseph Slone, Megan Cook, Cody Cook, Corey Cook, Trevor Wainscott and Trenidy Resor; and his sister Candy Hayes. He is also survived by 10 grandchildren.
