He was predeceased by: his former wife Joan M. Kosakowski (Troy); his sisters, Genevieve Kosakowski and Constance Lebida; and his father Julius Kosakowski. He is survived by: his wife Sophie Kucharski Kosakowski; his children, Patricia D'Amico, Lawrence Kosakowski and Thomas Kosakowski; his daughter-in-law Kelly; his grandsons, Frederick D'Amico, Jr. and Derek Kosakowski; his great grandchild Carson Kosakowski; and his nephews, Robert, Roger and Jessie (Colleen).
