He was predeceased by: his parents, Frank Middendorf and Madeline Middendorf (Graf); his sister Jane Krems; his children, Michael Middendorf, Clare Adams and Ruth Middendorf; and his grandson Ethan Middendorf.
He is survived by: his wife Patricia; his sister Ruth Regensberger; and his children, Christina Smith, Caroline Middendorf, Mark Middendorf, Matthew Middendorf, Martin Middendorf, Max Middendorf, Cheryl Skead, Marshall Middendorf, Mack Middendorf, Marcus Middendorf, Maurice Middendorf and Marcel Middendorf. He is also survived by many grandchildren, great grandchildren, nieces and nephews.
