She is survived by: her daughters, Jill Frederick, Jerri Bourgholtzer and Jan DeLong; her grandchildren, Richard Borte (Jadah), Jamie Frederick (Jeffrey Murphy), Zane Frederick, Angela Miley (Danny) and Jarred Bourgholtzer (Rachel); her great grandchildren, Chelsey, Benjamin, Olivia Borte, Blake and Jack Miley; and her nieces, Dawn Anderson (Brian) and Michelle Wilkins.
The family requests that in lieu of flowers, tribute donations in Jackie's name be made to the Alzheimer's Association.
