He was predeceased by: his wife Beth Hazuka (Riggs); and his parents, John P. Hazuka and Elizabeth Hazuka. He is survived by: his children, Denise Koontz (Frank Celin), John Hazuka (Dena), Cindy Kacin, Lori DeRosa (Don) and Melissa Kacin (Todd); his sisters, Bernice Pivirotto (Lou) and Margaret Koerner (Murray); his grandchildren, James Koontz, Megan Keel, Jordyn Rounkles, John Hazuka, Blade Gundermann, Cameron Hazuka, Michael Kacin, Brian Kacin, Justin DeRosa, Brandon DeRosa, Matthew DeRosa, Todd Kacin Jr. and Alexa Kacin; and his great grandchildren, Emma, Kaytlin Koontz, Robert, Maximus, Emmett Keel, Lily, Parker Kacin, Wesson Kacin, Logan DeRosa and Wyatt Rounkles. He is also survived by nieces, nephews, other relatives and friends.
