We are sad to announce that on November 19, 2018, at the age of 81, Addie P. Forsyth (Ponce Inlet, Florida), born in Geneva, Florida passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She is survived by: her parents, Thomas Walker and Hazel Clara Prevatt; her son Thomas (Mary Kay Hendrickson); her grandson Brandon; and her brother Carlton Eugene Prevatt (Paulette).

In lieu of flowers, the family request that donations be made to the American Lung Association.
