He is survived by: his parents, Michael Kolacz and Jeri Hamburg Kolacz; his wife Tina Mendoza-Kolacz (Wife); his children, Juancarlos Kolacz, Alex Kolacz and Matthew Kolacz; and his sister Jennifer Melcher.
