He was predeceased by: his parents, Mildred Ernestine Chalk and Harold Wesley Chalk. He is survived by: his daughter Tina Renee Chalk; his wife Helen Chalk (Jackson); his grandchildren, Ashley Keyes (Austin) and Wesley Dean; his nephews, William "Bill" Putman and Daniel Putman (Penney); and his niece Melissa Putman-Cox (Cory).
A celebration of life was held on Tuesday, September 10th 2024 from 4:00 PM to 6:00 PM at the Duh (501 N 9th Ave, Pensacola, FL 32501).
