He is survived by: his daughter Tina Renee Chalk; his wife Helen Chalk (Jackson); his grandchildren, Ashley Keyes (Austin) and Wesley Dean; his nephews, William "Bill" Putman and Daniel Putman (Penney); and his niece Melissa Putman-Cox (Cory).
