He was predeceased by: his father Edmond Kusiak; his wife Juliane Pollack Kusiak; and his sister Diane. He is survived by: his sons, Chris Kusiak (Holley), James Kusiak, Brian Kusiak and Jonathan Kusiak; his grandchildren, Michael Anthony Kusiak and Jennifer Lynn Kusiak; and his brother Jerry Kusiak (Marie). He is also survived by extended family & friends.
