He was predeceased by: his parents, Ernest Joel Fields and Winifred Marie Fields; his former wife Judith Rawlings Fields; his siblings-in-law, Jerry (Don), Gisela and Vernon (Isla); and his nephew Steven Sykes.
He is survived by: his daughter Judy; his siblings, Colonel Robert Ernest Fields (Gisela) of San Antonio, Texas. and Marie Fields Evans; his daughters, Lori Fields Newman (Peter), Carolyn C. Kahl and Suzanne Marie Fields (Bobby); his grandchildren, Peter John Thomas Newman (Brianna), Taylor Nicole Newman, Erin Aleigh Newman, Abigail Marie Kahl Olcese (Phillip) and Madeleine Anne Kahl (Jane); his great grandchild Emma Anne Olcese; and his sisters-in-law, Eunice Linden Ray and Celesta Linden Johnson. He is also survived by many nieces, nephews, and friends.
In lieu of flowers, the family asks those who are able and desire to do so, to honor Jack with a donation to St. Thomas Episcopal Church Endowment Fund and/or the Palm Coast Choral Arts Society.
