She was predeceased by: her parents, Mabel Marie Grudier and Bert Leslie Glaze; her siblings, Roy L. Glaze, Ted Glaze, Doris Merle Glaze, Evelyn Marie Glaze, Robert Douglas Glaze, Edward David Glaze and Becky Glaze; and her pets, Lilly and Willy.
She is survived by: her husband Paul Jakubowski; her children, Steven Henderson (Erin Henderson) and Lisa Kelley; her grandsons, Tim Cook Jr., Travis Callahan (Michael Donahue) and Todd Cook; her great grandchildren, Calianna Cook and Carter Cook; her siblings, Loretta Glaze (Bill Hendrock), Virginia Lee Glaze (Ray Smith), Richard Eugene Glaze (Belinda Glaze), Sandy Lyn Glaze (Ed Kisselburg) and Judy Ann Glaze (Tom Gehman); and also Hope Reid. She is also survived by many nephews, nieces, cousins, in-laws and friends.
In addition to her many hobbies and passions, Faye volunteered her time to several causes and was deeply involved with the American Cancer Society.
