He was predeceased by: his parents, Mitchell Crook and Noda Crook; his wife Marian Crook; and his siblings, Terry Crook, Wayne Crook, Gary Crook and Alice Lindsey.

He is survived by: his children, Donna Breeden, Debra VanAlstyne and Gilbert Crook (Becky). He is also survived by nine grandchildren; eighteen great-grandchildren; and one great-great-grandchild.

Flowers are welcome and may be sent to Fountainhead Funeral Home, or in lieu of flowers, donations may be made to VITAS Community Connection in Doug's memory.
