Carla Eloise Chadwick Obituary
We are sad to announce that on January 6, 2025, at the age of 79, Carla Eloise Chadwick of Orange Park, Florida passed away. Leave a sympathy message to the family on the memorial page of Carla Eloise Chadwick to pay them a last tribute.
She was predeceased by: her parents, James Chadwick and Leona "Phyllis" Chadwick; and her siblings, Cynthia Anfinson and Lawrence Chadwick. She is survived by: her siblings, Martha Vance and Paul Chadwick; her children, John Mihay Jr., James Mihay (Suk), Jennifer Atchison Boner (Paul) and Samantha Glass (Larry); and her husband Jack Cassidy. She is also survived by many grandchildren, great grandchildren, nieces, and nephews.
A celebration of life will be held on Saturday, February 8th 2025 from 1:00 PM to 4:00 PM at 2030 Wells Rd, Orange Park, FL 32073. It is necessary to RSVP via call or text to (904) 422-5678 with your name and number of attendees before February 1st.
