He is survived by: his wife Carmen Rosario; his children, John Rosario (Monica), Rafael Rosario Jr. (Deann), Bridget Rosario-Vega, Susan E. Rosario (Felix), Peter A. Rosario (Rebecca), Carmen G. Rosario (Michael) and Sherri Melissa Rosario; and his siblings, Luis Rosario (Santano), Daniel Rosario (Chique), Jose Juan Rosario and Delia Rosario. He is also survived by 25 Grandchildren, 16 Great grandchildren.

In lieu of flowers, memorials preferred to the Alzheimer's Association.
