He is survived by: his wife Michelle "Chelle"; his daughters, Amber (Chris Blackwell) and Katrina; his niece Lorrie Pittman; his grandson Jackson; his siblings, Linda Cisko and Mike Cook; his sisters-in-law, Kelly Milam (Terry), Janet Walsh and Paula Cook; and his nieces and nephews, Lisa Cisko, Laura Padgett (Ted), Katie Kylap (Chris), Ross Pittman, John Paul Walsh, Alex Walsh, Michael Avery (Rebecca) and Jonathan Avery.
