He is survived by: his child Dale, Jr. (Joanna); his grandchildren, Daniel and Katie; his son Nicholas Chlumsky (Lisa); his grandchildren, Alexandra, Nicholas and Lauren; his great grandchild Christopher Chlumsky; his sister Pat Stevens (Paul); his nephews, Paul, Jr. (Alicia) and Peter; his wife Mary Ann Simms; his stepchildren, Patricia Somara, Carrie Simms, Kelly Gilbert and Katie Patterson (Greg); and also Conrad Chlumsky (Kari Jo).
A memorial service will be held on Saturday, February 1st 2025 at 10:00 AM at the First Presbyterian Church of Naples (250 6th St S, Naples, FL 34102).
