He was predeceased by: his parents, Leroy Ballard and Eunice Ballard; and his siblings, George Ballard, Bill Ballard and Jeanette Parker.
He is survived by: his wife Cynthia Ballard; his siblings, O. C. Ballard, Jane Creech and Melva LeBrun; and his grandchildren, GraceLynn Villa, Maria Rosales and Jace Bryan Broxson.
