He was predeceased by: his parents, John W. Slack Sr. and Rebecca Orr; his grandparents, John, Margaret Slack, Harvey and Minerva Orr; and his son Brian.

He is survived by: his significant other Sherrie; his daughters, Paige (Tim) and Kristen (Matt); his brothers, Jim (Karen) and Gary (Eleanor); and his grandchildren, Heath, Aaron, Casey and Alex.
