She was predeceased by: her parents, Carnie Lee and Flossie Provau (Tuten); her husband David D. Phillips; and her brother Bennie Provau. She is survived by: her children, Stephen D. Phillips of Jacksonville, Kathy Padgett (Randy) of Middleburg, Vicki A. Prevatt (Michael) of Glen St and Amy L. Foley (Robert) of Taylor; her siblings, Ernie Provau (Peggy) of Seville, Minnie Gross (Don) of Jacksonville and Barbara Lipps (Danny) of New Symrna, Tennessee; and her grandchildren, Lisa (Bobby), Tina (Jeremy), Brandon, Michaela, Dalton (Katelyn), Mendle and Nicholas. She is also survived by 10 great-grandchildren; one great-great-grandchild; along with numerous nieces, nephews, other family members, and friends.
