He was predeceased by: his father Calvin Cook, Jr.; and his brother Dwight Cook. He is survived by: his wife Terry L. Cook; his mother Joyce Cook; his sons, Jeremy Cook (Jennifer), Brendon Cook (Danielle), Richard Stanke and Caleb Cook; his siblings, Brenda Cook, Debbie Thomas (Danny) and Mikell Cook (Bonita); and his granddaughters, Lucy Cook and Ruby Cook.
