He was predeceased by: his parents, James K. Polk and Adell Polk (Mattair); his wife Helen Polk; his grandson T. J. Polk; his great grandchild Anthony Polk; and his brother Ronald Polk.
He is survived by: his sons, Stephen Polk, Jr. and Theodore Polk (Angela); his caregiver Mary Angerson (Bill); and his siblings, James, Larry Polk, Christine, Lois, Wanda and Linda. He is also survived by nine grandchildren, ten great grandchildren.
