He was predeceased by: his parents, Howard Peter Underwood and Ona Underwood (Hill); his children, Daniel Underwood, Julie Elawag and Kim Runyun; and his grandson Omar Elawag.

He is survived by: his wife Susan Diane Underwood (Potter); his son Jeffrey Underwood (Cheryl) of Dallas, TX; his grandson Jamal Elawag; and his sister Linda Clark of McPherson, KS..

In lieu of flowers, donations are encouraged to be donated to Cornerstone Hospice.
