He was predeceased by: his siblings, Tom Hazzard, Frank Hazzard and Priscilla Webber. He is survived by: his wife Janet; his children, David Hazzard and Laurie Leonard of Maryland; and also, Donna Hazzard of North Carolina, James of Illinois, Mark, Melanie of Maryland, Paul, Heidi of Florida and Mike Leonard of Maryland.
In Lieu of flowers, the family requests that memorial be made in DeWitt's honor to the American Heart Association or to St. James Episcopal Church.
