She is survived by: her sons, Jakob Cook of Lake Wales, Jackson Cook (Emily) of Tampa and Parker Cook of Lake Wales; her stepparents, Mike Smith of Lake Wales and Vicki Gailey of Lake Wales; her sisters, Kelly Chandley (Matthew) of Lake Wales and Jennifer Christian (Rob) of Gainesville; her stepbrother Jeffrey Blankenship (A'lisa) of Trenton; her grandmother Charlene Lord of Lake Wales; her aunt Michelle Bixler of Lake Wales; her cousin Whitney Myers of Lake Wales; and the father of her children James Matthew Cook.
