He was predeceased by: his parents, Cecil Polk and Margaret Polk (Beverly Dawn Polk). He is survived by: his children, James "Jimmy" Polk of Mulberry and Deborah "Sissy" Perry (Al) of Clewiston; his stepsons, David Moore (Nancy) of Kennesaw Ga, Jerry Moore (Carol) of LaBelle and Tracy Farr (Gloria) of Ft. Myers; and his sisters, Carolyn Skipper of Zolfo Springs and Faye Granger (Lloyd) of Mulberry. He is also survived by 12 grandchildren and 12 great grandchildren.
