He is survived by: his wife Visy Cook of Lake City; and his children, Jesse of Lake City, Ivan of Lake City, John Cook of Lake City, Rachel Shaheen (Nathan) of Lake City, Renee Saferite (Lee) of Lake City and Elizabeth Shaheen (Joel) of Lake City.
