He is survived by: his parents, Joseph Johnson and Maria Johnson of Bristol, CT; his sister Laura Trabucchi (Peter Trabucchi) of Newington, CT; and his aunts and uncles, Antoinette, Don Cain of Stroudsburg, PA, Gerard and Joan Apruzzese of Murrells Inlet, SC. He is also survived by his godparents, and many cousins and friends.

Memorial contributions can be made to St. Jude Children's Research Hospital and/or the nonprofit organization Angel Flight, where Jay dreamed of piloting those in need (https://www.angelflight.com).
