We are sad to announce that on October 8, 2021, at the age of 58, Cindy Osborne Middleton of Jacksonville, Florida passed away. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

She was predeceased by: her parents, Philip Osborne and Mary Kelly Osborne. She is survived by: her husband Shawn Middleton; her daughters, Alicia Counihan (Josh) and Tiffany Milton (Chris); her grandchildren, Deacon, Paisley, Autumn and Archer; and her siblings, Lynn Howard (Glenn) and Robert Osborne (Robin). She is also survived by nephew and several nieces.

In lieu of flowers, contributions may be made to Isaiah 117 House of Duval County.
