He is survived by: his grandmother Peggy Hendrix; his father Layne Cook; his sister Tami Cook; his children, Sydney Carmichael, Faith Gotelli and Hope Cook; and his grandchildren, Mason Carmichael, Sophia Carmichael and Lori Gotelli.
