He was predeceased by: his parents, Alvin E. Bazley and Amelia C. Bazley; and his brothers, William S. and James A.. He is survived by: his wife Elaine C. Bazley; his son Tom Bazley (Cathy); his granddaughters, Ellen and Carolyn; his daughters, Maria Smith (Darryl) and Teresa Bazley (Holly); his brothers, John A. Bazley (Nancy) and Robert D. Bazley (Pat); and his siblings-in-law, Joan Bazley (William S.), Jean Bazley (James A.) and C. LaRue Colgan (Betty). He is also survived by many nephews and nieces.

In lieu of flowers, the family suggests making donations to St. Jude Children's Hospital.
